Liability Waiver and Release
By checking the box below, I acknowledge and agree that my child, named above, will be participating in vigorous athletic activities at the California Champ Camp, which involve inherent risks of serious injury, including permanent disability or death. I understand and accept these risks on behalf of my child and all parties associated with them.
In consideration of my child being allowed to participate in the California Champ Camp, I agree that we voluntarily assume all risks associated with participation. I understand it is my responsibility to ensure that my child, or I, inspect the facilities and equipment prior to participation. If anything appears unsafe, we will immediately notify a coach or supervisor and refrain from participating until the issue is resolved.
I acknowledge that these activities may involve risks arising from my child’s own actions or inactions, the actions or inactions of others, the rules of play, the condition of the premises, or the use of equipment. I further understand that there may be risks not known to us or not reasonably foreseeable at this time. Nevertheless, we accept full personal responsibility for any injury, loss, or damage that may result from participation.
By checking the box, I release, waive, and discharge the California Champ Camp, Ceres High School, Ceres Unified School District, Ceres High School Wrestling Club, Central Valley High School, Ceres Pups Wrestling Club, and all affiliated organizations, including their directors, administrators, coaches, agents, employees, sponsors, volunteers, advertisers, facility owners, and lessors (collectively referred to as "the Releasees"), from any and all liability for injuries, including death, or damage to property, whether caused by negligence or otherwise.
I confirm that I have read and fully understand this waiver and release, and I agree to its terms voluntarily by checking the “I agree” box.
